An authorized individual from the business must submit the Redemption Authorization Form telling us which cardholder(s) have redemption rights.

If you have been reissued an SNB Visa CheckCard, you will need to register your new CheckCard on the UChoose website, even though you previously registered your old card. The registration process is the same and you will not lose your reward points from your old debit card. If you also link other SNB accounts to aggregate your reward points, then you will need to re-link those accounts to aggregate reward points.
